Privacy Policy

An important message to all our customers

The confidentiality of individuals’ health information is strongly protected in the health sector – through the obligations health service providers have under professional and ethical codes of practice. At Cook, Dillon, Brudenell, individuals’ rights to privacy have always been important to us. We are supplying this leaflet because CDB holds personal information in connection with the services it provides to patients.

Consent

The Privacy Act 2001 states that consent can be express or implied. Express consent is given explicitly, either orally or in writing. Implied consent is agreement that can be inferred from an individual’s conduct.

What information do we hold?

Our records include information that identifies you (like your name and address) and other information supplied by you and by other people on your behalf, such as on the Health Questionnaire form and letters from referring dentists. During the course of our relationship with you, we may gather information about you relating to other services that we provide. This may be in the form of x-rays and diagnostic casts.

How may we use this information?

We may use your personal information:

  • only for the primary purpose for which it was collected, or for directly related secondary purposes if these fall within the reasonable expectations of the individual
  • in other ways if the individual gives consent (whether express or implied)
  • where required or authorised by law
  • where necessary to prevent or lessen a serious or imminent threat to someone’s life, health or safety
  • for research provided certain conditions are met
  • to help us provide the correct dental advice and consultation at the time of your initial visit with us
  • to efficiently manage all orthodontic treatment and services provided to you
  • to ensure that our internal business operations are running smoothly which may include fulfilling all legal requirements and conducting confidential systems maintenance.

What rights does CDB have to disclose personal information?

Depending on the service that we provide you, we may disclose your personal information to:

  • Referring dentists or oral surgeons
  • A “person responsible” for an individual who cannot give or communicate their consent.
  • Any external service providers that provide services to us. This is on a confidential basis and may include organizations that provide laboratory services or for purposes of debt recovery.
  • Additionally, we will disclose information if we are required by law to do so or if the law allows us to do so or if you provide your consent for us to do so.

Your authority

By continuing to accept orthodontic services from Cook, Dillon, Brudenell, you consent to CDB maintaining, using and disclosing your personal information as set out in this leaflet.

What are your rights?

You can request access at any time to the personal information we hold about you. If you believe that any information is incorrect or out of date, you may, of course, ask us to correct it. There is no fee for requesting access to your information, however, we may charge you the reasonable cost of processing your request.
